    Job Description
    The Community Long-Term Support Worker provides support and service coordination to children living with an intellectual/developmental disability, physical disability, and/or severe emotional disturbance and their family/caregivers with the primary responsibility of promoting the child’s health, safety, and welfare in their home and community. Based on 37.5 hours per week, full-time, non-exempt. 2026 Salary: $28.96 - $31.31 per hour Minimum Qualifications Qualifications may be met through a combination of education and practice experience including: An associate’s degree in a human services-related field and two years of experience working with people of the specific target group(s) for which they are employed. A course of study and practice experience that meets requirements for state certification or licensure as a social worker and also one year experience with individuals with disabilities. A course of study leading to a Bachelor of Arts or Bachelor of Science degree in a health- or human services-related field and one year of experience working with individuals with disabilities. A minimum of four years’ experience as a long-term support SSC. A combination of training and experience equivalent to four years of long-term support case management practice.
